Saint Elizabeth Community offers Aging Life Care Services to help families map out their plans for the future. Our RNs and Care Managers connect aging adults with everything from scheduling home care to finding a nursing home. Our staff are experienced at listening, evaluating and advising so we can connect the dots between clients, families and providers to ensure high-quality care.

Individualized Plan

Here's how a licensed care manager will develop an individualized care plan for you

  • Assessing needs
  • Identifying goals
  • Making recommendations for care
  • Connecting clients to services
  • Follow up to assess changes over time

Services provided by Licensed Care Staff

Doctor communication: We will accompany clients to appointments as needed and will implement any recommended changes, new medications, follow-up tests, and appointments. We will coordinate the pick-up and pre-filling of medications.

Medication management: Registered Nurse reconciles medications with attending doctors and pre-fills medicine trays for a weekly or bi-weekly period. Initial review for possible drug interactions and side effects is made and communicated with doctors.

Assistance with long-term care insurance: Care managers can review policies related to home care services, confirming waiting periods and required documentation.

Crisis management: We will assist in emergency situations; with respite coverage or in the case of hospitalization have appropriate staff go to the hospital at family's request.
